You will decide who can access your health information and assist you in these decisions. During your appointment, you can expect your doctor and our staff to speak directly to you about your health, plans, lab results and questions. While you may still be on your parents' insurance, it will be your responsibility to make and keep appointments, to manage your immunizations and medications, and to understand your health insurance and options.
Our website will help you through this process, step by step. The first step will be getting you signed up for MyChart. This will allow easy communication and access to your health information.

We encourage you to discuss any questions or concerns with your pediatrician as they help you complete the Portable Medical Summary, which can be shared with your new adult care team at your first visit.
