For Teens
Let's talk about taking control of your health! When you turn 18, you will be legally responsible for your healthcare. You will decide who can access your health information and assist you in these decisions. During your appointment, you can expect your doctor and our staff to speak directly to you about your health, plans, lab results and questions. While you may still be on your parents' insurance, it will be your responsibility to make and keep appointments, to manage your immunizations and medications, and to understand your health insurance and options.
Our website will help you through this process, step by step. The first step will be getting you signed up for MyChart. This will allow easy communication and access to your health information.
For Parents
Let's talk about helping your child take control of their health! We are here to help you and your child build the skills they will need to transition to the adult healthcare world. Each teen's plan will be unique and personal. Our website has tools, discussion topics and questions to ask your teen and their doctor to determine readiness and timing. For Parents of Children with Disabilities and Chronic Conditions
We recommend Lurie's Chronic Care Transition program. Lurie understands that switching from pediatric to adult care can be a challenging adjustment for patients with disabilities and chronic conditions. They provide programs and support for teenage patients making sure they have continued access to high-quality health care throughout their adult lives.

Parents and teens can review this helpful Checklist prior to their 18 year well visit or earlier if appropriate. We encourage you to discuss any questions or concerns with your pediatrician as they help you complete the Portable Medical Summary, which can be shared with your new adult care team at your first visit.

MyChart is not meant to replace the advice given to you by your clinician, and should not be used to diagnose a health-related problem or to substitute for medical care. MyChart messaging is not in real-time; allow at least two (2) business days for a response. If your child is having an emergency, call 911. If you think your child may need to be seen, call your child's clinician or 1.800.543.7362 (1.800.KIDS DOC).
MyChart Access for Teens
Complete this form to sign up for your MyChart account. Your request will be routed to your pediatrician to complete setting up your account, An email confirmation will be sent to you within 2 business days. Kids First Pediatrics does not have access to MyChart; but you may fill this form out to assist in gaining access for other medical providers responsible for your medical care.
MyChart Access for Parents
Parents can complete this form to request MyChart access for their children. If you are requesting access for your child/ren who is/are 12 or older, you will also need to submit a signed authorization form for Enhanced Access. Learn more about the difference between Standard and Enhanced access to your child's information. Kids First Pediatrics does not have access to MyChart; but you may fill this form out to assist in gaining access for other medical providers responsible for your medical care.
An important step of the process of transitioning to an adult provider is making sure your insurance is aware of which provider you will be seeing and removing your affiliation with your Lakeview Pediatrics provider. To change your provider, you must select your new PCP (primary care provider) with your insurance company.
